Mgen Contact Tracing

If you perform in the adult industry and have been diagnosed with Mycoplasma genitalium (Mgen), you can use this form to provide PASS with the information we need to help keep the industry safe. PASS uses contact tracing to identify, notify, and monitor individuals who may have been exposed to an infectious disease in order to prevent further infections. Filling out this form is entirely voluntary, and the information you decide to provide is up to you.
